The Challenges of Living with Metastatic Cancer
Cancer that spreads from where it started to a distant part of the body is called metastatic cancer. For many types of cancer, it is also called stage IV (4) cancer. The process by which cancer cells spread to other parts of the body is called metastasis.
Metastatic disease typically manifests after a protracted period of undetectable disease following surgery or systemic therapy, owing to relapse or recurrence. In the case of breast cancer, metastatic relapse can occur months to decades after initial diagnosis and treatment.
In the past, many people did not live long with metastatic cancer. Even with today's better treatments, recovery is not always possible. But doctors can often treat cancer even if they cannot cure it. A good quality of life is possible for months or even years.
Nearly all types of cancer have the potential to metastasize, but whether they do depends on a number of factors. Metastatic tumors (metastases) can occur in three ways: They can grow directly into the tissue surrounding the tumor. Cancer cells can travel through your bloodstream to distant locations in your body.
Stage 4 cancer has spread from its original location to distant parts of the body. It's sometimes referred to as metastatic cancer. This stage may be diagnosed years after the initial cancer diagnosis and/or after the primary cancer has been treated or removed.

The lungs. The lungs are the most common organ for cancers to spread to. This is because the blood from most parts of the body flows back to the heart and then to the lungs. Cancer cells that have entered the bloodstream can get stuck in the small blood vessels (capillaries) of the lungs.
A cancer journey can be overwhelming.
Some cancers tend to remain in place and not grow much at all. Others grow slowly—so slowly that they may never require treatment. Aggressive cancers may grow and spread so quickly that they metastasize before the cancer has been diagnosed.
Stage 2A breast cancer
One of the first ways cancer starts to metastasize from its original location is by spreading into the lymph nodes. The lymph nodes most likely to be affected are in the armpit (called axillary lymph nodes). Stage 2 breast cancers typically spread to up to three lymph nodes.

In some situations, metastatic cancer can be cured. But for most metastatic cancers, treatment does not cure the cancer but it can slow its growth and reduce symptoms. It is possible to live for many months or years with certain types of cancer, even after the development of metastatic disease.

These include: spread via lymphatic channels – this is favoured by most carcinomas. spread via blood vessels – this is favoured by sarcomas and some carcinomas that originate in the kidneys - because of their thinner walls veins are more frequently invaded than arteries and the spread is via veins.
