Botox is a great tool for lifting heavy upper lids and sagging eyebrows. I see many patients in their 20's, 30's and 40's that are bothered by a slight drop of the brow and heaviness on the upper eyelids.
Usually, a small amount of the Botox spreads to the nearby central area of the frontalis muscle. This makes it harder to raise the inner eyebrows. However, the outer part of the frontalis muscle is working normally. So the outer eyebrows are lifted and you see a peaked appearance.
If you are experiencing eyebrow unevenness this can also be corrected, and you do not have to wait for the Botox injections to wear off. Eyebrow unevenness can be corrected by injecting a little more neurotoxin into the side that is lower. This will eventually correct the asymmetry.
Heavy brows and the feeling of drooping.
This feeling and look comes from treatment of the forehead muscles called the frontalis. These muscles move the brows up and in certain cases after Botox treatment, the relaxed muscle then makes the brows drop.
By elevating the eyebrow slightly, Botox lifts the upper eyelid and reveals a small amount of eyelid skin. Botox is a short-term solution for treating hooded eyelids. The neurotoxin's effects will gradually wear off, and the eyebrow muscles will regain their strength.
Eyelids may droop after a Botox procedure if the provider administers too much Botox or injects it in the wrong location. In rare cases, the neurotoxin spreads to the wrong muscle causing eyelid drooping when the patient doesn't follow the provider's aftercare instructions, such as lying down or bending over.
Completing the Procedure
The placement of injections and disbursement of the Botox are both very important in order to achieve the best results. For most patients, between 4-8 units is common for the outer brow and 20-30 units on average for the area between the eyebrows.
When Botox is applied to problem areas around the eyebrows, the muscles relax and the skin on top of them becomes smoother. The muscles around the eyebrows are pulled upwards, elevating the eyebrows and making a patient's eyes appear more open. Patients are thrilled to find they look alert, peppy, and rested.
Therefore, by injecting Botox into the corrugator supercilia and orbicularis oculi, you can relax the muscles pulling the eyebrows down which will in turn emphasise the strength of the frontalis muscle. By injecting botulinum toxin into the lateral part of the orbicularis oculi muscle, the brow can be raised and arched. As a result, the brow can appear more lifted; the eyes may appear more open.

Further botulinum toxin treatment
The next option for correction, which surprises many patients, is that you can treat an eyelid ptosis, even one caused by botulinum toxin injections, with MORE botulinum toxin! The eyelid, like most moving structures in the body has muscles which oppose each other.
To accomplish this, BOTOX may be injected into the outer corners of the brows and the area between the brows, known as the glabella. This technique aims to force the relaxation of the muscles that are responsible for drooping brows, creating the illusion of a raised upper lid.
It raises the brow for a few millimeters (at least 4 mm to 6 mm) to open up the eyes. Typically, an oculoplastic surgeon may prescribe an average dose of 12 to 24 Botox units for treatment around and under the eyes.
If the neurotoxin is injected too close to this small muscle, it will be weakened and the eyelid cannot be opened. This starts showing 2-4 days after injection but is more pronounced after 2 weeks when Botox takes full effect.
The first “true” results will be noticeable within 24 to 48 hours – when you'll see a softening of wrinkles. Why does Botox need a couple days to start working? Botox freezes your muscles by stopping the release of acetylcholine.
Patients seeking brow lifts are typically between 40- 50 years old and are in good general health. Patients in their 20s and 30s might only require cosmetic surgery if they have inherited brow drooping or gone through a serious injury.
While brow lift can achieve more dramatic changes, Botox is preferable for making slighter, more subtle improvements. The timeline for each procedure is also much different. Botox results last about three to four months in this area, whereas brow lift changes are generally visible for a decade.
