A red portal will appear if the player has also killed the final boss on that difficulty. If playing classic, that means killing Diablo, but if the expansion is installed, the player will have to kill Baal before the portal can be opened.
You can open the Red Portal to the cow level as many times as you like, providing you don't get credit for killing the king.

The tristram portal is always open once you opened it. Just head over to the stony field to find the portal go in and grab the leg. It's gonna be there every game. When someone makes a cows game, the first thing they do is go to tristram to get the leg again.
