As a young Jedi Knight, Anakin lost a hand in a battle with Count Dooku, which received a robotic replacement. He later returned the favor by removing both of Dooku's hands. Eventually, he lost both his legs and his remaining arm in the climactic duel on Mustafar between both he and Obi-Wan.
Always one to outdo itself, the saga would go on to give Anakin the most brutal dismemberment of all: his battle with once-beloved mentor Obi-Wan Kenobi resulted in him losing his legs and remaining organic arm.
Darth Vader lost his left arm and both legs when Obi-Wan Kenobi defeated him on Mustafar. After turning to the dark side of the Force, Skywalker severed Mace Windu's right hand to save Darth Sidious when Windu tried to kill the latter.

Unfortunately, these needles, which transmit neurological data throughout his body and allow him to control his limbs, also cause him constant pain. Between the never-ending itchiness and walking around with needles in his skin, it's no wonder he is so angry.
Eventually, he lost both his legs and his remaining arm in the climactic duel on Mustafar between both he and Obi-Wan. As Vader, he had each limb replaced with cybernetic parts, but would lose his robotic right arm one last time when Luke Skywalker faced his father in Return of the Jedi.
In the duel, Anakin lost both of his legs, his one remaining organic arm, and suffered (possibly up to) fourth-degree burns across what was left of his body that were severe enough to damage his internal organs.
After Count Dooku severed the right arm of Anakin Skywalker in the opening battle of the Clone War, a mechno-arm was grafted to what was left.
In the aftermath of the slaughter, Vader dueled his former Jedi Master, Obi-Wan Kenobi. Vader lost the fight after having both legs and his left arm severed by Kenobi. Vader then tumbled down the volcanic planet's surface, near a river of lava, where his robes burst into flames, consuming his entire body.
He can only breathe without the need for his mask when he is in one of his meditation chambers seen in The Empire Strikes Back. This environment is pressurized and controlled enough to survive inside it without using his helmet, and it's the only time he can eat normally.
It evolved over the years but it never stopped. Palpatine was constantly grooming, manipulating and emotionally abusing Anakin. He presents himself as a kind, accepting figure that gives Anakin what he so desperately craves (acceptance and approval). Palpatine was grooming Anakin to become Darth Vader.
So, all of his limbs are, in fact, intact. However, Kylo Ren didn't walk out of that fight squeaky clean. We do know that Kylo is sporting a brand-new scar, which makes him even more like his granddad Darth Vader.
During his confrontation with Obi-Wan Kenobi on Mustafar, Anakin Skywalker was first maimed by lightsabre cuts. He lost both legs around the knees, and much of his left arm.
The canonical comic Star Wars: Darth Vader #7 confirms the younglings' deaths caused Anakin grief, self-loathing, hatred and pain, fueling his descent even further into the dark side. Those negative emotions also power the dark side, meaning Anakin likely felt stronger as he killed the younglings.
The choice to leave his mother at a young age continues to haunt Anakin throughout his life. He is plagued by nightmares of her and visions of her fate. When he eventually saves her from a group of Tuskens on Tatooine, his blind anger at her dying state consumes him.
ANAKIN: I won't let this one become real, Padmé. This marks the beginning of Anakin's obsessive fear about losing Padmé. He's not being entirely unreasonable, though; the last time he had these terrible dreams was when his mother was being tortured by the Sand People, and he got there too late to rescue her.
Anakin Skywalker/Darth Vader lost his right hand to Count Dooku on Geonosis in Attack of the Clones. He lost the replacement cybernetic on Nelvaan in the Clone Wars micro-series (Legends). He lost his left hand and both legs to Obi-Wan Kenobi on Mustafar in Revenge of the Sith.
Hayden Christensen, who starred as Anakin, sported two braids in the film that were used to show his character's rank as a padawan within the Jedi Order. Had Anakin become a Jedi Knight — and, you know, not Darth Vader — the braids would have been cut off to designate his new rank.
Anakin foresaw and feared Padme's death in childbirth.
The original spark behind Anakin's turn was the nightmare that saw his wife dying in childbirth. He thought the Dark Side would grant him the power to prevent that reality from coming true.
Even after his brutal scorching in the fires of Mustafar, sand remained the only natural phenomenon that Vader feared. To him, sand was a reminder that he'd failed to save his mother or Padme from death.
Darth Vader also experimented with Force healing fueled by the dark side, in which meditation on anger and frustration at his life and circumstances allowed his lungs to function without the aid of his hyperbaric chamber or his life support suit.
