There are no biological children after Brianna in Outlander
Claire and Jamie don't have any more children after Brianna. Claire heads through the stones while pregnant with Brianna at the end of the second season/book. She spends 20 years in the 20th century, and she and Frank don't have any children together.
Claire Fraser was born on October 20, 1918.
That would make her 27-and-a-half at the time she and Jamie meet in the books, while Jamie's barely 22. This would make Claire five and a half years older than Jamie.

Surprise! Jamie also has two step-daughters, Marsali MacKimmie Fraser and Joan MacKimmie. They became his step-daughters when he married their mother, Laoghaire MacKenzie. If that name sounds familiar, it's because Laoghaire is the same woman who orchestrates Claire being put on trial for witchcraft.
So, it seems Jamie went through with the marriage in a bid to build a life by himself without Claire. Also, he chose Laoghaire's daughters rather than her because of his desire to have children and be a father.
However, in the series, Jamie did know about her involvement as Claire told him. As she was recovering in the wilderness with Jamie, she explained how it had been Laoghaire who was behind it all.
"He fell in love when she broke down and wept in his arms and let him comfort her," the author says. Jamie falling first was inevitable since he didn't have the complications of having a secret double life and spouse in the future like Claire.

This, together with the sight of Brianna's photograph – stolen by Geillis from Jamie's collection – having been burned in some sort of ritual, confirms that Geillis intends to travel through time again and find Brianna, whom she now believes to be the heir of Lovat's line.
Because yes, Claire and Lord John end up married in book seven, and yes, they drunkenly (and full of grief for the loss of Jamie, whom they both love with all of their heart) consummate that marriage.
Jenny meets Brianna Randall Fraser for the first time at Lallybroch, when the girl travels into the 18th century to search for her parents. Jenny is delighted at seeing that Jamie fathered a daughter as they had never expected him to have children, and marvels at Brianna likeness to him.
It helps to remember the whole timeline, which is easier when you read the books. When Jamie met /married Claire, he was a 23 year old virgin, and they were together three years before she went back through the stones pregnant with Brianna in 1746.
When asked about the situation, Outlander author Diana Gabaldon confirmed that Claire did not use ether in the books. Fans described the nurse using the drug to sleep as "out of character" for her and questioned why STARZ decided to turn her into a drug user.
