June 13, 1942: Tom Riddle murders Myrtle Warren (a.k.a. Moaning Myrtle) in the first-floor girls' bathroom at Hogwarts School of Witchcraft and Wizardry, sending a piece of his soul into his diary. It becomes his first Horcrux.
Discovered in chapter 16, The Chamber of Secrets
When Harry asks her how she died, Myrtle says she was crying in the bathroom, heard a boy's voice and saw a pair of large yellow eyes. She died instantly, and returned to haunt Olive Hornby, a girl who used to tease her.
Death. On her final day of her life in early June 1943, Myrtle was teased by student Olive Hornby, who made fun of Myrtle's glasses and left Myrtle alone sobbing. Myrtle then ran into one of the bathroom's stalls and started crying.
From what I understand, she spent her initial time as a ghost harassing her bully. It's possible she kept out of everyone else's sight so Dumbledore didn't realize she was available. Ghosts are also disinterested in the living world, so getting her to focus enough to answer questions would have been difficult.
Myrtle Warren was once a Hogwarts student, but after her untimely death, she stayed back as a spirit to eternally haunt the second-floor girls' bathroom. Myrtle had a hard time at Hogwarts, and was frequently bullied. Prone to wailing morosely, the ghost became known informally as Moaning Myrtle.
Moaning Myrtle
Born in the late 1920s to Muggle parents, Myrtle Warren started her Hogwarts life in the early 1940s, where she was sorted into Ravenclaw. She had no friends, and was bullied relentlessly because of her glasses and acne.
circa 1928-1929 – 13 June, 1943), more commonly known as "Moaning Myrtle", was a Muggle-born witch who attended Hogwarts School of Witchcraft and Wizardry from 1940 – 1943 and was sorted into Ravenclaw. She was killed in 1943 by Salazar Slytherin's Basilisk, under Lord Voldemort's (Tom Riddle) orders.
She was just 14 when she died.
It's not surprising to me that she stayed on as a ghost. She was so young and hadn't had the chance to experience much of life.

At that time, he had been turned into a Death Eater and was tasked with killing Dumbledore on behalf of Voldemort. In a moment of transparency, Draco breaks down in the bathroom and explains the situation to Myrtle, who listens intently. Afterward, the two become good friends.
Riddle ultimately changed his name, by way of an anagram, from "Tom Marvolo Riddle" to "I am Lord Voldemort", as a way of removing his own heritage and of exalting himself, and he killed the Riddle family, partly because they were Muggles, and partly because they abandoned him and his mother.
The most likely reason why Lord Voldemort's nose disappeared and he devolved into such an evil-looking creature is that as he dabbled deeper into the Dark Arts, his appearance gradually became as distorted as what remained of his twisted soul.
In June 1943, a student named Olive Hornby bullied Myrtle about her glasses, which led Myrtle to hide in the girls' bathroom where she could cry alone.
Moaning Myrtle Doesn't Look Like A Child
That's because Henderson was 36 years of age when she first played Myrtle, as the filmmakers ultimately decided to cast an older actress in the part to make their natural ageing seem less noticeable.
Harry Potter and the Chamber of Secrets (2002) - Shirley Henderson as Moaning Myrtle - IMDb.
After her death, Myrtle's ghost (or Moaning Myrtle, as the students usually call her) haunted Olive Hornby, the fellow student who caused her to be in the bathroom that evening. When the Ministry of Magic refrained her from doing so, Moaning Myrtle returned to the bathroom, haunting indefinitely.
The biggest revelation in Harry Potter and the Cursed Child was that Lord Voldemort had a daughter with Bellatrix Lestrange, who was named Delphini.
The cup, which was made a Horcrux with Hepzibah's death, was left with Bellatrix Lestrange who kept it at Gringotts. Griphook and an Imperiused Bogrod helped Harry, Ron and Hermione break into Bellatrix's vault and recover the Horcrux. The cup was destroyed by Hermione Granger, using a fang from the Basilisk's corpse.
Lord Voldemort's first Horcrux was created when he was still at Hogwarts, known then as Tom Riddle. After murdering a schoolgirl called Myrtle, with the help of the ancient monster that dwelled within the castle, the Basilisk, his soul was split for the first time.
Moaning Myrtle's full name was Myrtle Elizabeth Warren.
Shirley Henderson has been most widely-seen as Moaning Myrtle in the Harry Potter films- 2002's "Harry Potter and the Chamber of Secrets" and 2005's "Harry Potter and the Goblet of Fire." Henderson's tiny frame and raspy voice have led to her frequent casting as introverted eccentrics.
"Moaning Myrtle's full name was Myrtle Elizabeth Warren," the author tweeted on Monday.
Myrtle is a self-confessed pervert. She's at least 50 years old, but points out that in the movie, she tried to look at Cedric Diggory's private parts.
Apparently no one was interested enough to ask her, not even Dumbledore, who thought Hagrid was innocent. They were interested, however, in forbidding her ghost to haunt Olive Hornby, the witch who had made her life so miserable that she was crying in the bathroom when the Basilisk got her.
