As mentioned, Gramma Tala's health begins to decline once she gives Moana the Heart of Te Fiti. Considering the ocean chose Moana when she was a child, someone needed to keep the precious Heart safe.
Moana's grandmother told her she must restore the heart of Te Fiti because her island and all life is dying without the heart. To restore the heart she would need to find Maui and convince him to help her restore the heart.

Although Moana's relationship with Maui never extends beyond the platonic level in the film, and although he is a 1,000-year old demigod and she is a 16-year old human, a lot of fans of the film like to put her and Maui in a romantic pairing known as Hooked Wayfinder.

When Moana and Maui find Te Ka, a spirit risen due to the stealing of the heart of Te Fiti, they fail to get past her and Maui's hook is badly damaged. Upset and let down, Maui leaves Moana.
