Relapse rate was similar within the two groups. Five-year overall survival was 66.2% in the free flap subset and 58.8% in the no free flap subset.
Free flap surgery is considered a reliable and safe procedure for head and neck reconstruction. The success rate of a free flap described in the literature is about 95% 1-3.
In most medically fit patients, flap surgery which is major complex surgery is considered safe with a low complication rate.
Complications in free flap surgery may be attributed to numerous surgery-related risk factors: poor anastomosis technique [16], vessel size mismatch, poor-quality of recipient vessels, compression or twisting of anastomosis or pedicle, short pedicle necessitating anastomosis under tension, vasospasm of pedicle, ...
The majority of flap failures occur within the first 48 hours. With early recognition and intervention of flap compromise salvage is possible. Successful salvage rates range from 28% to over 90%.
About 3 out of every 100 (about 3%) flaps fail completely and the cells in the whole flap die. If this happens, you will need to have surgery to remove the flap. If your flap fails completely and is removed, you will need to recover fully from the operation.

A flap is any piece of tissue that is moved to cover a wound. A free flap is a piece of tissue that is disconnected from its' original blood supply, and is moved a significant distance to be reconnected to a new blood supply.
The major disadvantage of the free fibula flap bone is the reduced vertical height. The typical height of the free fibula flap is usually 13 to 15 mm, which is proximate to the normal height of the atrophic edentulous mandible and almost half of the height of a dentate mandible [1].
Signs of a congested (venous compromised) flap include a bluish color, increased swelling and warmth, and shortened capillary refill (<2 seconds). In the case of ischemia (arterial compromised flap), the flap will be pale in color, cold, and will have a delayed capillary refill (>3 seconds).
DIEP flap or tissue flap breast reconstruction can be performed immediately following a mastectomy or as delayed reconstruction. Patients will be given general anesthesia before surgery. The surgery takes four to six hours, but it can take longer if performed along with a mastectomy.
Advantages for Patients
One of the main advantages of free flap surgery is the ability to restore appearance and function to larger areas. Also, free flap tissue does not naturally contract in the same way that skin graft tissue does.
It is common to experience some pain following gum flap surgery. Some people also get anxiety issues about their perception of pain after the surgery.
Still, flap surgery is not without its risks. For example, the procedure may cause the tooth roots to become more sensitive or the contour or shape of the gums to change. Gum surgery may also introduce harmful bacteria into the bloodstream and put gum tissue at risk of infection.
Skin flaps are thought to provide better cosmetic results than skin grafting (Fig. 5), as the skin tone and texture are usually better matched. Additionally, they have a reduced chance of failure in comparison to skin grafts. However, flap failure remains a potential complication of the procedure*.
Slotted flaps are popular on modern aircraft because they are the most efficient type of flaps on the market; they provide the most combination of lift and drag in terms of aerodynamics. A slotted flap increases in airplane's wing camber, which means that the curve of the leading edge to the trailing edge rises.
The flap procedure is necessary when severe gum disease (periodontitis) has damaged the bones that support your teeth.
Particularly in the first few weeks after surgery, sleeping on your back, while usually wearing a surgical bra, allows the breasts to heal in a more comfortable and desirable position.
Discomfort: Slight swelling of the operated area is not unusual. Chapped lips or bruising of the lips / cheek area may occur. A reusable ice bag or a frozen vegetable bag, wrapped in a soft towel, may be applied to the area of surgery to help minimize the swelling of your face.
The skin graft will usually be held in place using stitches, staples, clips or special glue. The area will be covered with a sterile dressing until it has connected with the surrounding blood supply, which usually takes around 5 to 7 days.
Results: Twenty-six patients were included in this study. The amount of blood lost during the procedure ranged from 6.0 to 145.1 mL, with an overall mean of 59.47 ± 38.2 mL.
You will need to undertake some lifestyle modifications based around showering and bathing for 2-3 weeks after surgery and avoid strenuous activity for around 3-4 weeks after surgery to ensure the graft site is not stretched or injured.
