If you have a delay in your milk coming in, don't feel discouraged. Continue to express milk. That means removing milk from your breasts with a breast pump or by hand. And continue to breastfeed often, even if you are supplementing with formula for a few days.
Your healthcare provider would likely recommend that you continue to pump until your baby is able to breastfeed effectively and once you have a sufficient milk supply. Once this has happened, they will likely advise you to stop pumping and just breastfeed - and, of course, get plenty of rest in between.
Perhaps you have polycystic ovary syndrome (PCOS), a low or high thyroid, diabetes, hypertension (high blood pressure) or hormonal problems that made it difficult for you to conceive. Any of these issues may also contribute to low milk supply because making milk relies on the hormonal signals being sent to the breasts.
For the first 2 to 5 days after your baby is born, you will make a small amount of colostrum. This is the early, thick, rich milk that is high in nutrients. Colostrum is all a healthy full-term baby needs. Around 3 to 5 days after birth, your milk will come in.
Generally, moms should be pumping every 3 hours. Pumping more often can help stimulate breasts to produce more milk. Moms can try pumping both breasts for 15 minutes every two hours for 48-72 hours. Then moms can return to their normal pumping routine. Pumping for longer than 30 minutes may not be beneficial.
Despite views to the contrary, breasts are never truly empty. Milk is actually produced nonstop—before, during, and after feedings—so there's no need to wait between feedings for your breasts to refill.
Why it happens: Low milk supply can be the result of not pumping enough, pumping too soon after a feeding or not nursing between sessions. You may also have a low supply if you're not pumping long enough, if you're using the wrong type of pump or you're not drinking enough liquids.
When does breast milk come in? Mothers usually notice their milk coming in two to three days after the birth. Around this time full or engorged breasts replace the small volumes of colostrum that are already present. Milk supply continues to increase as long as the baby—or hand expression or pump—empties the breasts.
Adequate hydration also is important for breast milk production. The amount of liquid you put into your body affects how much breast milk you can produce. I encourage women to carry a bottle of water for themselves in their diaper bag.
You're not getting let down. If your breasts feel like they're full but you're not able to get the milk flowing out when you pump, it could be that you're not achieving let down. The let down reflex releases your milk from the milk ducts. This only occurs when you're either breastfeeding or pumping.
These sessions don't need to be evenly spaced, but you should be nursing/pumping at least once during the night in the first few months or anytime you notice a decrease in supply. Avoid going longer than 5-6 hours without pumping during the first few months.
Something I recommend to moms is the 5-5-5 rule. Try and use milk within five hours at room temperature, five days in the fridge, and by five months in the freezer.

Breastfeeding or offering formula in a bottle are not the only ways to feed your baby. Moms can decide to exclusively pump and give their baby all the benefits of breastmilk without having to nurse.
Even if more time has passed, it may still be possible to increase your milk production to some extent, but unfortunately there is no guarantee of being able to make a full milk supply. Any amount of milk you are able to produce is valuable to your baby.
The fluid your breasts produce in the first few days after birth is called colostrum. It's thick and usually a golden yellow colour. It's a very concentrated food, so your baby will only need a small amount, about a teaspoonful, at each feed. Your baby may want to feed quite often, perhaps every hour to begin with.
When your breasts are empty, they will probably feel lighter and no longer uncomfortably full, as they might have at the start of the pumping session. You can also pick them up to see if they still feel heavy or full of milk.
About 10-15 days after birth, you start making mature milk. Like each phase of breast milk, it has all the nutrients your baby needs. The amount of fat in mature milk changes as you feed your baby. Let your baby empty your first breast before switching to the other breast during a feeding.
Stress doesn't directly affect milk supply. The amount of milk your body makes depends on how often your baby nurses. The more milk he or she drinks, the more your body will make.
One of the best ways to increase breast milk production is to make sure you aren't suffering from dehydration. Remember, dehydration can dramatically decrease breast milk production. By staying hydrated and avoiding dehydration, your body will have the water and electrolytes it needs to build milk supply.
