Once Gravelord Nito is dead and you're back at Firelink Shrine, run back the stairs towards the elevator. You might have to pull the lever to make the elevator come up. Once down, go into the second tower and towards the Valley of Drakes.
The shortcut to Lost Izalith is in Demon Ruins just outside the room where the Demon Firesage boss was encountered. Facing the room, look to the right at a hole in the wall. Use the tree roots to reach the ground floor and then go to the end of the long hall to find a locked door.

After you defeat The Bed of Chaos, come back to the shortcut passage again and you'll find Solaire of Astora sitting down in it, all depressed about something, but the next time you see him he'll be available to summon for the final boss fight.
Bed of Chaos is the best boss in the game. Seriously, I love the way the fight can just flip and suddenly you're on the losing side. It's so well designed, and makes excellent use of Dark Soul's jumping mechanic.
Solaire goes insane and attacks you near the door to the Bed Of Chaos Boss Fight. Solaire goes insane and the next time you visit the shortcut area, he can be found there, hostile. Solaire shows up as normal (albeit depressed) just outside of the shortcut door and you can talk to him to trigger his final Summon Sign.
