Does Luffy fall in love with Hancock? Luffy does know, but he doesn't care because he doesn't have the same feelings for her. While he is deeply grateful to her for everything she has done to help him and respects her for the hardship and trauma she's faced, he doesn't love her romantic or sexually.
One of my biggest pet peeves is the idea by some that Luffy's rejection of Hancock — the most beautiful woman in the world — is a consequence of him being childish, stupid, and/or ignorant of love and marriage. What Oda has done with Hancock is brilliant.

Whenever Boa Hancock was shown on-screen, fans were awestruck and went crazy for her charms. As a result, Boa is considered the most beautiful woman in the One Piece universe.
Clearly, Vivi has a deep love and admiration for the Straw Hat crew. However, that's where the relationship ends. There was never anything that deeply personal between Luffy and Vivi. Plus, Vivi literally has her childhood friend, Koza, acting as a more direct and consistent relationship.
Robin also seems to be very devoted to Luffy, as he is the only person whom she wants to get stronger for. This shows how great her dedication to her captain has become after being rescued from Enies Lobby. Upon learning of Ace's death, Robin was very shocked and became greatly concerned about Luffy's wellbeing.
Monkey D. Lily is Luffy's legitament heir. She is the daughter of Monkey D. Luffy and Boa Hancock.

Unfortunately for Hancock, the romance is one-sided, as Luffy does not share the same feelings for her. He is entirely focused on his journey to becoming the King of the Pirates. Luffy trusts her as a comrade and friend but has never stepped beyond these limits.
