It meant they wouldn't be able to have any kids (because they're the last of the human race) and they would only have each other for the rest of their lives and they couldn't transcend after they die, but they were alive, together, and finally able to live in peace.
Notes and Trivia. In "The Last War", humanity achieves transcendence with the following exceptions: Clarke Griffin is the first being to have ever committed murder in the test. Because of this, the Judge does not allow her to transcend.

Bellamy, for example, dies before humanity takes their final test, so, despite his belief, he doesn't transcend. Since Clarke won't transcend either, she will die a natural death.

Emori physically did die, [but] her mind was still 'alive' in the mind space, so she transcended.
By the end of The 100, she is either 33 or 34. Octavia's physical age by the end of the show is ultimately higher than Bellamy's or Clarke's. The decade she spent aging with Hope and Diyoza on Skyring makes her older than Bellamy who is around 29 or 30 years old in the last season of The 100.
As all The 100 fans know, Clarke will do anything for the people she loves, and this time that meant Madi, not Bellamy. So in one of the biggest twists of the entire series, Clarke shot and killed Bellamy in order to save Madi.
Six years later, Clarke's Nightblood is revealed to have saved her from the death wave and she has adopted a young Nightblood named Madi, the only other survivor Clarke could find on the planet.
Madi is a young Nightblood who runs into Clarke in Eden as the only Praimfaya survivors on the ground. After gaining each other's trust, Clarke takes Madi under her wing and raises her as her own child.
Clarke sacrifices herself and stays on Earth, but her Nightblood allows her to survive. Clarke survives in the only inhabitable land left on Earth, Shadow Valley, with her daughter Madi, another Nightblood she found after Praimfaya.
Clarke still has the key to the City of Light in her head, as it helped her survive getting her mind wiped. This would imply that the Disciples want that chip to help recreate the City of Light, and Clarke is the key in that she's how they'll create a perfect world without war.
In the sixth season, after 125 years in cryosleep, Clarke, Bellamy, and the others wake up to find out that they are no longer orbiting Earth and have been brought to a new habitable world, Alpha, also known as Sanctum.
What Clarke did at Mount Weather not only weakened Lexa's command (and made the Ice Nation emboldened to rise up for war), but it also made Clarke incredibly strong. So strong that they're calling her Wanheda, which is Trigedasleng for "Commander of Death."
While the show is only loosely based on the books and has now far surpassed any territory explored in the source material, it bears mentioning that Bellamy and Clarke do have a romantic relationship in the books on which the show is based. They eventually get engaged, and it's implied they build a home in the woods.
In February 2015, Bellamy began dating the American model Elle Evans. They announced their engagement in December 2017 and married on 10 August 2019. Their daughter, Lovella Dawn Bellamy, was born in June 2020.
Before 54 bc Octavia was married to Gaius Marcellus, by whom she had two daughters and a son. On the death of Marcellus in 40 she was married to Mark Antony, who at the time was ruling the Roman state with Octavian and Marcus Aemilius Lepidus.
But just before Emori knocks out Murphy, she whispers “due north.” How romantic. Jaha uses that faith he's so fond of to trust Murphy's belief that Emori was telling the truth.
The only reason Madi didn't go back was because she knew Clarke wouldn't want her to grow up with no one her age.
Octavia, Raven, Murphy, Emori, Indra, Gaia, Levitt, Jackson, Miller, Niylah, Echo, Hope, and Jordan have all chosen to live the rest of their lives with Clarke on Earth. And that's the end—a happy ending.
Though the two characters were close for years and years on the show, their connection remained platonic until the end, when Bellamy met a deeply unsatisfying fate. But the two actors (who are married in real life) have a new project, and this time, they're definitely a couple—a married one.
Those who do make the choice to stay include Raven, Murphy, Emori, Echo, Nylah, Hope, Jordan, Octavia, Levitt, Miller, and Jackson. In other words, these are the characters who survive.
Temporal Flare
Diyoza gets out, but she realizes that Octavia has no time. She tells her she will return. Octavia submerges herself in the quicksand-like substance and survives the Temporal Flare with only damage to a hand that was left exposed.
