The heist goes awry; the comedian leaps into a chemical vat to escape Batman, surfacing disfigured. This, combined with the earlier accidental death of his wife and unborn child, drives the comedian insane, turning him into the Joker.
Following several adventures with Catwoman and Ivy, Harley betrays them and breaks into Arkham Asylum, intending to kill the Joker for his years of abuse towards her.

Joker is also cured of his insanity in this story, which makes him a good man who actually becomes a council member in Gotham City. No longer a villain, Joker is now a person who wants to stop the corruption in the city -- think Harvey Dent before he became Two-Face in The Dark Knight.

While the two worked together for years, the only one keeping any semblance of a relationship alive was Harley. Truthfully, the Joker was never that interested in Harley, save as someone he could easily leave holding the bag during his criminal endeavors.
Aside from the two names stated above, Joker referred to the illness as Joker-itis and Mad Clown Disease (a parody of hepatitis and Mad Cow Disease). Batman mentioned that Joker Infection was a variant of Creutzfeldt–Jakob disease that was mutated beyond any medical record.
The psychopathology Arthur exhibits is unclear, preventing diagnosis of psychotic disorder or schizophrenia; the unusual combination of symptoms suggests a complex mix of features of certain personality traits, namely psychopathy and narcissism (he meets DSM-5 criteria for narcissistic personality disorder).

The Joker was abusive toward lovelorn Quinn: hitting her or throwing her out of buildings, all while playing sadistic mind games with her. The series clearly depicted the patterns of domestic violence: He beat her and cast her out before wooing her back.

These relationships could have made him feel safe and protected and buffered his trauma, but instead were limited and empty.

Why can't Joker be cured? It's too bad Joker's brain is like a raging storm and he can't be permanently fixed - because in the issue, it appears the Joker can be redeemed. Although, it took all of Martian Manhunter's power to cure him, even for a few minutes - so, clearly, there's no easy long-term cure for the Joker.
