Jamie doesn't really help matters by choosing this moment to confess that he has cheated – but not with Malva. Instead, he spent one night with someone else before going to prison, long ago (and when separated from Claire by 200 years).
Malva developed a sexual relationship with Ian Murray during the time frame that she was trying to find someone to place blame upon for her pregnancy. Ian knew that he was not the only man she was sleeping with, after he showed late for one of their meetings he found her having sex with Bobby Higgins instead.
The idea of Jamie having an affair seems ludicrous. Claire eventually realizes that Malva is lying because she knows her husband well enough. Tom seems to question Malva's story. You can see it in his face that he's trying to figure out if this story is true.
During Outlander Season 6, Episode 6, Claire's near-death experience was the least of her problems. Months later, the Christies came to the Frasers with news. Malva is pregnant and she claims that Jamie is the father.

In the TV series, we get to see how Malva knows all about Jamie's scars. She spies on Claire and Jamie when they're in the barn, which is going to lead to all of his scars on show. The TV show gave us this scene so that it would clear up questions later on.

Jamie's Secret is a story about a nine-year-old who loves soccer, playing with her best friend Sadie, and collecting crystals. She loves hugging and climbing trees, watching butterflies, and soaring on her swing. Jamie loves life and everything that points to it.
After starting off the season fairly well, Claire ended it imprisoned for Malva's murder in a rat-infested jail in Wilmington, injured from being stoned, her hair chopped off, and no idea whether Jamie is dead or alive. Her only ally in Wilmington is Tom Christie, and he's a dubious ally at best.
After Malva accused Jamie of cheating on Claire, Claire and Jamie spoke privately in the barn on Fraser's Ridge and discussed the accusation, and much of Jamie's storied past, all the while omitting a key detail.
However, while Malva is killed in May 1775 in the book's timeline, it isn't until November 1776 before her killer is revealed. Eventually, Claire finds Allan Christie by Malva's grave and he admits to having sexually abused his sister since she was a child and the unborn child was actually his.
