It really boils down to two errors in judgment in particular that highlight the Jedi Master's failure. First, Yoda dismissed another Jedi's premonitions about the return and rise of the Sith. Jedi Master Lene Kostana's primary mission was to learn about the Sith so as to prepare the Jedi Order for the Sith's return.
The Empire had Stormtroopers searching everywhere for Jedi in hiding. Had Yoda attempted to rebuild the Jedi Order, all those Force-sensitives together would've been sensed by Sidious, who would've sent the 501st to deal with them, most likely assisted by Vader.
While Yoda sensed that the Sith's plan involved the Clones, it was too late to prevent Order 66. Instead, he placed his faith in the Force and allowed the dark events to happen.

The most likely explanation, however, is that like the Jedi did in so many other ways, Yoda underestimated the strength of the dark side. He probably thought Anakin had the strength to resist the darkness and help bring down the Sith.

Yoda was reluctant to train Anakin in Episode II because he said Anakin was too old to start training (even though Anakin was still a boy). However, in Episode V, Yoda didn't seem very reluctant to train Luke despite the fact that Luke was a full grown man by that time.
