Some people have strokes without realizing it. They're called silent strokes, and they either have no easy-to-recognize symptoms, or you don't remember them. But they do cause permanent damage in your brain. If you've had more than one silent stroke, you may have thinking and memory problems.
Weakness, numbness or paralysis in the face, arm or leg, typically on one side of the body. Slurred or garbled speech or difficulty understanding others. Blindness in one or both eyes or double vision. Vertigo or loss of balance or coordination.
People who have mild strokes may feel like they've dodged a bullet since physical symptoms—blurred vision, difficulty speaking and weakness or numbness on one side of the body—usually disappear in a few minutes. Remember, if you notice any signs of a stroke in anyone young or old, seek immediate medical attention.
Call 9-1-1 immediately if any of these signs of stroke appear: Numbness or weakness in the face, arm, or leg; Confusion or trouble speaking or understanding speech; Trouble seeing in one or both eyes; Trouble walking, dizziness, or problems with balance; severe headache with no known cause.
“If you have an inability to speak words, notice food or liquid falling out of your mouth due to facial droop, you could be having a stroke. If you experience numbness in your face or extremities, can't move an arm or leg or have a sudden onset of double vision or dizziness — those are other possible stroke symptoms.”
Some of the most common stroke mimics are seizures, migraine, fainting, serious infections and functional neurological disorder (FND). Once the person is diagnosed, they can have treatment or support to manage their symptoms.
Brain scans and tests
You should have a brain scan soon after symptoms start, within an hour of arriving at hospital if possible. The scan can show whether the stroke is due to a clot or a bleed. There are two main types of scan used: A computed tomography or CT scan.
A person's life expectancy after a mini stroke reduces by around 4% in the first year following the attack in comparison to people who have not had one. In the following 9 years, life expectancy reduces by 20%. These statistics come from a 2019 review .
The longer a stroke goes untreated, the greater the chance for even more extensive brain damage and disability. Just because the stroke is over, that does not mean that the brain damage is put to a halt. In fact, ischemic strokes unfold over a period of 10 hours.
If a stroke is suspected, a CT scan is usually able to show whether you have had an ischaemic stroke or a haemorrhagic stroke. It's generally quicker than an MRI scan and can mean you're able to receive appropriate treatment sooner.
Some people will experience symptoms such as headache, numbness or tingling several days before they have a serious stroke. One study found that 43% of stroke patients experienced mini-stroke symptoms up to a week before they had a major stroke.
An MRI can also detect old strokes for decades after they happen. The fastest type of MRI is diffusion-weighted imaging (DWI). It measures shifts in fluid in the brain and can detect a stroke soon after its onset. An MRI can also detect evidence of past strokes.
Introduction: An infarct on brain MRI is often seen as gold standard when diagnosing ischemic stroke. Although MRI has high sensitivity in detecting a lesion shortly after ischemic stroke, this rapidly declines when time progresses.
Pre-strokes or mini strokes are the common terms used to describe a transient ischemic attack (TIA). Unlike a full blown stroke, a TIA only lasts a few minutes and does not cause permanent damage. Nevertheless it is a warning sign that a possible stroke may be coming in the future.

Blood tests for stroke. There is no blood test that can diagnose a stroke. However, in the hospital, your doctor or nurse may do a series of blood tests to learn the cause of your stroke symptoms: Complete blood count (CBC).

The easiest way to differentiate between the two is to pay attention to sensations. A migraine headache produces sensations like auras, flashing lights, or tingling skin, while a stroke-related headache causes sensations to be lost, such as a loss of vision or feeling.
