Because fasting can elevate cortisol levels. One of cortisol's effects is that it raises blood sugar. So, in someone with blood sugar regulation issues, fasting can actually make them worse.
For humans, cortisol begins to increase immediately after fasting commenced (Fig. 1C) [78]. Five-day fasting increases cortisol levels and shifts the peak from the morning to the afternoon [79]. Other fasting experiments for 2.5 to 6 days dramatically elevates plasma cortisol levels [80–82].

Higher cortisol levels linked to abdominal obesity
Higher long-term cortisol levels are strongly related to having abdominal obesity, according to a 2018 review study. However, not all people with obesity have high cortisol levels.
Medications to control excessive production of cortisol at the adrenal gland include ketoconazole, mitotane (Lysodren) and metyrapone (Metopirone). Mifepristone (Korlym, Mifeprex) is approved for people with Cushing syndrome who have type 2 diabetes or glucose intolerance.
There are numerous ways to help reduce cortisol levels and combat potential weight gain. The most effective methods include exercising, practicing mindful eating, ensuring sufficient sleep, and practicing meditation.
Cortisol acts on the liver, muscle, adipose tissue, and pancreas. In the liver, high cortisol levels increase gluconeogenesis and decrease glycogen synthesis.
Cortisol is associated with increased appetite, cravings for sugar, and weight gain in both animal and human studies [8].
