The episode ends with Alicent backing away, the camera lingering on the Catspaw Dagger that we saw Viserys wield earlier in the episode, and the king finally dying, reaching out one more time and seemingly speaking to his dead wife, Aemma, when he says, "My love."

The show has got the speed of the disease correct. King Viserys has lived with leprosy for many decades and the disease has grown very slowly.
His legendary ancestor, King Aegon the Conqueror, had a vision that the world of men would fall, and understood that only his family of Aryan inbreds could save it. Viserys tells his daughter, Princess Rhaenyra, that Aegon “foresaw the end of the world of men.
Frankly, Viserys didn't tell Daemon about Aegon's dream because he didn't trust him with it or believe he would give it the dedication it required. It's part of why Viserys never made Daemon his Hand or his heir, as Daemon was far more concerned with personal ambition than his duty to the realm and humanity.
Viserys Comes to Court After a Desperate Plea From Rhaenyra
She repeats what Viserys told her, that it is their duty to hold the realm united against a common foe, though granted that foe is still a couple hundred years away from attacking. But without that foresight, she notes that naming her heir divided the realm.
The reality is that Viserys was simply attempting to reaffirm what he told Rhaenyra, what she believes, and what he continues to believe. But what happened is that Alicent now believes—and you can even understand where she's coming from—that Viserys' dying wish was for Aegon—their Aegon—to be king.
With no one left to help her, Aegon II demanded his dragon Sunfyre to kill and then consume his older sister right in front of the eyes of her last remaining child and future king, Aegon The Younger.

To Daemon, Aegon's Song of Ice and Fire is trivial and perhaps mean nothing, but conquest through dragon power means everything. Daemon choking Rhaenyra after learning about Aegon's dream is also based on his resentment that Viserys never told him.

In many ways, for Rhaenyra who has always felt the pressure of the realm preferring her younger half-brother over her, this would have been a solid confirmation that even in his last moments, Viserys does not regret naming her as his heir.
