It is safe to fly with high blood pressure if the medical condition is well controlled. However, if your hypertension is extremely high and unstable, it is best to avoid boarding a plane. A consultation with your doctor before flying is primordial to know if you are fit to fly.
If your blood pressure is higher than 120 over 80, seek medical advice before you travel.
Research has found that exposure to very high altitudes of 5,400 meters could possibly lead to an increase of 14 mmHg in systolic blood pressure and 10 mmHg in diastolic blood pressure within a 24-hour period. As mentioned before, this increased blood pressure can also lead to fatal cardiac arrest if not taken care of.
If it's still very high, seek medical care. Call 911 or emergency medical services if your blood pressure is 180/120 mm Hg or greater and you have chest pain, shortness of breath, or symptoms of stroke. Stroke symptoms include numbness or tingling, trouble speaking, or changes in vision.
Your blood pressure could rise
The higher you are in the sky, the less oxygen your body will carry, and less oxygen means higher blood pressure. If you typically have a regular blood pressure or even a low blood pressure, this increase will likely have no effect on you.
The air pressure while flying increases the risk of blood clots forming, which is not ideal. Heart complications: If you've suffered a heart attack, stroke, cardiac failure or chest pain while at rest, flying should wait. The lower oxygen levels, increased air pressure, high altitude and more could affect your health.
Your blood pressure is considered high (stage 1) if it reads 130/80. Stage 2 high blood pressure is 140/90 or higher. If you get a blood pressure reading of 180/110 or higher more than once, seek medical treatment right away. A reading this high is considered “hypertensive crisis.”

People with altitude illness should not travel to higher elevations until they no longer have symptoms. A person whose symptoms get worse while resting should travel to a lower elevation to avoid becoming seriously ill or dying. High-altitude cerebral edema (HACE) is a more serious form of AMS.
The available scientific literature suggests that above 3500–4000 m, the risk of developing stroke increases, especially if the exposure is acute among non-adapted populations.
Call 9-1-1 immediately if any of these signs of stroke appear: Numbness or weakness in the face, arm, or leg; Confusion or trouble speaking or understanding speech; Trouble seeing in one or both eyes; Trouble walking, dizziness, or problems with balance; severe headache with no known cause.
In other words, once blood pressure rises above normal, subtle but harmful brain changes can occur rather quickly—perhaps within a year or two.

Anxiety doesn't cause long-term high blood pressure (hypertension). But episodes of anxiety can cause dramatic, temporary spikes in blood pressure.

In the United States, there are 0.07 fatalities per billion passenger miles, which translates like this: If you fly 500 miles every day for a year, you have a fatality risk of one in 85,000. In short, flying is, by far, the safest mode of transit.
