During the conflict, the use of Juyo caused many Jedi to fall to the dark side and join forces with the Sith. As a result, the Jedi Council restricted the use of the Form, and in the time following the end of the conflict, the Jedi battlemaster Skarch Vaunk trained Juyo to only a few hand-picked students each year.
Form IV: Ataru
As lightsaber wielders started becoming more skilled and powerful in the force, Form IV, also known as Ataru or the Aggression Form, grew in prominence. Ataru fighting techniques date back through the Old Republic and even were employed during the Mandalorian Wars.
Maul's main form appears to be Niman, Form VI. In the canon card game Star Wars: Destiny – Spark of Hope, Maul is credited as a master of Niman, which makes sense since the Form is mainly used by wielders of double-bladed lightsabers and those who employ Force powers mid-fight.

As Darth Tyranus, Dooku trained both Asajj Ventress and Grievous in Form II. Makashi evidently became one of Grievous' primary fighting methods, as many of his opponents commented on Dooku's influence in his technique, and he passed this training on to his IG-100 MagnaGuards.

Form Zero was a principle followed by many Jedi, not exactly a form of lightsaber combat despite its name. The principle of Form Zero was that a Jedi's most important martial skill was to know to only draw one's lightsaber when absolutely necessary.
Ahsoka Tano was also skilled in the use of Jar'Kai and wielded both a lightsaber and a shoto. She employed an extremely complicated Ataru-Shien reverse grip style to her Jar'Kai tactic that was worthy of mastery.
Mace specialized in the seventh form of lightsaber combat, AKA Juyo or Vaapad. Windu was the only known practitioner of this form that didn't fall to the Dark Side, mainly because the style calls for an aggressive, unpredictable approach (much like Maul's, just less jumping around).
