As a young Jedi Knight, Anakin lost a hand in a battle with Count Dooku, which received a robotic replacement. He later returned the favor by removing both of Dooku's hands. Eventually, he lost both his legs and his remaining arm in the climactic duel on Mustafar between both he and Obi-Wan.
Levi is right. If you watch in slow-motion, you can see Obi-Wan sever Anakin's left forearm first. Obi-Wan then twists around while Anakin is still in mid-somersault and swings through the exposed legs. I just re-watched the scene and it takes roughly 1.5 seconds for Obi-Wan to complete both cuts.
In the duel, Anakin lost both of his legs, his one remaining organic arm, and suffered (possibly up to) fourth-degree burns across what was left of his body that were severe enough to damage his internal organs.

1 Darth Sidious

Thought dead, Darth Maul survived his injuries by focusing on his hatred of Obi-Wan Kenobi, the Jedi who cut him in half. His shattered body was dumped amid the refuse of the junk planet Lotho Minor, where the once deadly warrior fell into madness, staying alive on a diet of vermin.
Later stripped of his black Nightbrother markings, Maul was left solely red-skinned. Agonizing Sith tattoos were applied personally by Darth Sidious in an attempt to cleanse Maul's mind as well as demonstrating Maul's complete dedication to the dark side.
Visions of Obi-Wan. The comic opens during the last moments of Revenge of the Sith, recreating the well-memed instance when a despondent, freshly-armored Vader cries out with a drawling “nooooooo” over how much he's lost.
Darth Vader's Suit Embeds Needles In His Skin
Inside his suit and mask, Vader has a series of needles in his skin. Unfortunately, these needles, which transmit neurological data throughout his body and allow him to control his limbs, also cause him constant pain.
The redeemed Anakin Skywalker accomplished the state of immortality, as well as his children, Luke Skywalker and Leia Skywalker Organa Solo, years later.
Outcome. The Duel on Cloud City was a lightsaber duel that took place between Luke Skywalker and Darth Vader, during the Galactic Civil War. The inexperienced Skywalker was ultimately outmatched against Lord Vader and was defeated, losing his hand in the process.
