Reva knows that Anakin Skywalker is Darth Vader because she was one of his targets—a child—during the Order 66 killings that took place during the events of Star Wars Episode III: Revenge of the Sith.
One of the primary antagonists of the show is an Inquisitor named Reva, who was also present at the attack on the Jedi Temple, and is aware of Darth Vader's identity as a result. She joined the Inquisitors so she could eventually take revenge on Vader.
